Application for Admission
Complete the online application to apply for admission to Minnesota State College Southeast, even if you have applied for admission to another Minnesota State college in the past. Complete the ENTIRE application or the registration process will be delayed. For more information, see: Step 1: APPLY - Admissions Process

Authorization to Release Student Info (Download PDF)
This form gives consent to Minnesota State College Southeast to disclose a student's educational records as specified by the student. Visit the college's Educational Records procedures for more information. To request a copy of your transcript, submit the Transcript Request.
Authorization to Withhold Directory Information (Download PDF)
At Minnesota State College Southeast the following is considered to be directory information and will be available to the public when you begin your classes unless this form is completed and returned to the Office of the Registrar by the fifth day of your starting semester:
- Student's name
- Major field of study
- Participation in officially recognized activities and sports
- Dates of attendance
- Graduation date
- Degrees and awards received

Use this form to update your student information. It is very important that your contact information be current so faculty and staff can keep you abreast of current information.
Child Care Grant Program Application (Download PDF)
This application with instructions can be downloaded from the Minnesota Higher Office of Education and is for assistance with child care expenses. A student needs to have completed the Financial Aid application, and eligible students need to be MN residents not receiving MFIP.
Complete a Company Billing Authorization form if employer is to be billed for student's tuition, fees, books, or application fee.
Consortium Agreement - Minnesota State College Southeast (Download PDF)
All students who are taking credits at another school who want to receive financial aid for those credits must complete this form. Please print off form, fill out and give to other college for completion.
Submit this form to request a conference to address special needs and to review the available services. It is imperative to Minnesota State College Southeast that all students with disabilities have equal opportunity and access to education and training.
Direct Deposit Authorization (Download PDF)
All students applying for Financial Aid must complete this form as part of the financial aid application process. Included on this form is Direct Deposit information. Any Financial Aid disbursements a student may receive will be directly deposited into the bank account on this form.
This form is required by the State for all on-campus students.
Minnesota State Grant Program - Student Eligibility Questionnaire (Download PDF)
This worksheet includes additional state grant questions needed to determine a student's state grant award. This worksheet should be completed by students who may be eligible to receive a state grant.
Preferred Name Request Form (Download PDF)
Complete this form if you would like Minnesota State College Southeast to use a preferred name for you in its records, consistent with System Procedure 1B.2 Preferred Name and Minnesota State College Southeast Policy 313 Nondiscrimination in Employment and Education Polices and Reporting Procedures. If you complete this form, your campus email address, system directories, class rosters and other Minnesota State College Southeast records will be changed as technically feasible except where your legal name is required. The completed form is to be turned in to the Registrar’s Office.
If you live in Wisconsin, South Dakota, Nebraska, Missouri,
Michigan or Manitoba, you are required to complete this reciprocity form.
If you live in North Dakota you are required to complete this reciprocity form.
Use this form to request a recommendation from a faculty member or supervisor for a potential work position, scholarship or honorary award and/or admission to another education institution.
Students and/or parents who did not sign the original FAFSA form are required to complete this form. The Financial Aid Office will notify you if you are required to complete this form.
Student Grievance Report and Procedure (Download PDF)

If you will not be parking a car in the college lot while a student, submit this waiver online.
Use this form when your instructor requires proctored testing and you cannot go to the Winona or Red Wing campus to complete these tests.
Substitution of Required Course (Download PDF)
This form is required when there is a need to make a substitution for a required course.
Use this form to request a Minnesota State College Southeast transcript ($7.00/request).
MSC Southeast has authorized National Student Clearinghouse to provide its online transcript ordering service. This service provides students with timely notices regarding their transcript request as well as order status history. Students will need a major credit card and a valid email address. Credit cards will be charged only after orders have been processed. The cost is $7.00 per transcript.
However, if you are applying to a Minnesota State Colleges and Universities (Minnesota State) institution and attended MSC Southeast since 1990, a transcript request form is not necessary. An electronic transcript will be available at the receiving institution upon application. Inform the college you plan to attend that you should have an eTranscript from Minnesota State College Southeast.

1. Go to the eServices login
2. Enter your STarID and Password (Click in the “Display Name” Box if you wish to have your name appear on any documents when printing.) Click on “Login”
3. Click on Grades and Transcripts
4. Click on Academic Record
5. Choose Chronological and click on "Get Academic Record." Your unofficial transcript will be displayed.
